The Nurse is Incredibly Sexy

I apologize if some parts feel unnatural, as I am translating from Japanese.

Here’s a guide to the monsters in UNDERWARD, including their traits and how to deal with them. As expected, nurses are the idols of the hospital! Since the monster names are currently unknown, I’ve identified them based on their appearance and location.

I have cleared up to the 16th floor (technically, I was carried through) but after the service stage on the 6th floor, the difficulty increases, and it feels like the stages start looping.

For example, the 7th floor feels like it’s actually the first floor with a significant increase in difficulty.

Monster

1・Nurse #1

The most threatening presence in this game. At first, it appears to be a sexy figure, but once it detects an intruder, it transforms into a grotesque form and chases after you. What’s troublesome is that even if you defeat it, it will respawn after a certain period of time.

If you’re not careful, it can respawn near where you’re carrying a capsule, leading to a counterattack. Fortunately, defeating it isn’t too difficult, and you can deal with it using your throwing items. Once it’s down, make sure to move away quickly.

2・Nurse #2

It’s often hard to spot because it’s on the ceiling, and you’ll usually realize it’s there by the sound of it getting closer. It has a wide detection range and will slowly approach if something is nearby, making it an annoying presence. It might be a good idea to set up ceiling-attached needles to deal with it. Like the others, it will respawn after a certain period of time.

3・In the locker

A creature in the locker. If you open it from the front, it’s game over as you’ll be eaten. The solution is to open the locker from an angle to avoid this.

Also, from the bottom of the locker where this creature is, red liquid is leaking out.

4・Ceiling

A creature with something resembling an anemone growing out of a vent. There’s a bloodstain-like spot on the ground, and as long as you avoid standing there, you should be fine… or so you’d think. This threat has swallowed many veteran players, as if it’s well-versed in human behavior.

Even if you crouch and move, it can still consume you, and while you can fend it off with throwing attacks, it will regrow after a certain amount of time. Over time, players tend to forget about it and become its next prey, making it a truly lethal anemone!

Well, since it doesn’t chase you, the best solution is just to stay vigilant.

5・IV stand

A monster that has parasitized the IV stand. It requires a bit of technique to hit, but focusing on the pole part of the IV stand should make it easier to target.

Since it doesn’t move, you can deal with it using throwing items.

Update 2025-02-10: The hitbox has been enlarged, making it easier to land a hit.

6・Stuffed Animal

A cute monster often found near experimental capsules. It intimidates with a surprisingly strong roar. Like the IV stand monster, it doesn’t move, so you can calmly use throwing items to deal with it.

I’ve also spotted elephant and baby versions. They don’t attack, but their loud cries could attract nurses.

7・CardBoard

A monster that has parasitized a cardboard box. It’s quick, but it doesn’t cause any harm. The box can be opened, and there are items inside. Sometimes it drops them, making it kind of cute.

Lately, I’ve started to get frustrated by how the box suddenly starts moving and surprises me.

8・Imitating Monster

Appears on the 4th floor (also on the 10th and 16th floors). This cunning monster respawns multiple times and seamlessly blends in with its allies to kill them. In single-player, you can tell it’s an enemy as soon as you encounter it, but in multiplayer, it can lead an inexperienced team to destruction, making it a formidable foe. It’s truly the wall of trials that stands in your way from the 4th floor onward.

You can identify it by the tentacles that appear when it attacks. It also screams “Help me!”

9・Found you!

It’s troublesome when you get possessed while acting alone. You can fend it off by quickly turning around and attacking. If it possesses a teammate, throw an empty bottle to help them. I haven’t experienced it yet, but apparently, this monster will start attacking after some time.

10・Giant Monster

A giant monster that appears only on the 5th floor (and the 11th floor). It breaks through walls, spawns in multiples, and respawns after a certain period of time. With extraordinary health and overwhelming power, it crushes everything in its path. It’s the second wall of trials that stands in your way as a team.

To defeat it, high-damage throwing weapons like grenades or Molotov cocktails are effective.

11・Nurse #3

A spider-like nurse confirmed on the 8th floor… but this one is no longer a nurse. It’s incredibly fast, and many players find themselves becoming its prey without realizing it.

On top of being strong as an individual, the environment from the 7th floor onward, where you need to light up the area to maintain visibility, makes it even more difficult than expected.

How to deal with it? I got eaten before I could even try.

It seems like the nurses will be progressively stronger from this point onward. They’re fast, really fast.

12・The Glow of Fireflies

Creatures that are unleashed when the time limit runs out. They’re variations of the nurses, but all of their abilities far exceed the norm, and even if you defeat them, they respawn almost immediately. It’s essentially like facing a game over.

Fortunately, there were a few instances where, if there was enough distance or if the whole team fought back together, we were barely able to clear the situation. Sometimes, if everyone works together, there may be a way out, so never give up.

The song played when the monsters are unleashed is the traditional Japanese tune “Hotaru no Hikari” (Glow of the Firefly).
